owed. This payment option will not delay the commencement of your assigned
effective date; however, claims will not be reimbursed until payment is
received.
Please write “Bill Me Later” on the last page of your application in the
Credit Card payment area should you prefer not to prepay with a check or credit
card. Fax toll free or mail your application. We will pay for the cost of your
